S1 Ep1: Acne Scars, Wrinkles, and Combination Treatments

In the first episode of LaserChat, Kathy Taranto, CMLI, CMLT, managing partner at Mint Aesthetics, and Erin Hennessey, DNP, FNP-C, owner of Blush Aesthetics, discuss their approaches to treating acne in a young patient.

Forever Clear BBL®, Sciton’s BroadBand light procedure, is highlighted for promoting clear and healthy skin. This pulsed-light device uses a blue light (420 nm) to target bacteria and yellow light to address post-acne redness, effectively clearing breakouts and stubborn acne.

Kathy performed three Forever Clear BBL treatments on her patient in the first few months, complementing BBL with alternate treatments, such as steam for opening and clearing out pores, extractions, and LED lights. This combination, Kathy says, provides her patients with immediate gratification and visible improvements in acne within just one to two treatments, encouraging them to maintain their clear skin with an at-home care skincare regimen.

When asked how Erin would treat this patient with acne, she proposes eight Forever Clear BBL sessions spaced over several months.

Once the acne calmed down, Kathy turned to ProFractional™, Sciton’s fractionated erbium laser, to address scaring, stating, “ProFractional is still one of my go-to’s for acne scarring—it’s so fast, and there’s no tip involved.” On the other hand, Erin favors HALO, noting, “HALO has become my go-to for acne scarring.”

S1 Ep2: Melasma, Sun Damage, and Ethnic Skin Types

In season 1, episode 2 of LaserChat, Dr. Jason Pozner, M.D., F.A.C.S., a board-certified plastic surgeon, founder of Sanctuary Plastic Surgery, and co-founder of Sanctuary Medical Aesthetic Center in Boca Raton, FL, joins Dr. Patrick Bitter Jr., M.D., a board-certified dermatologist and founder of Advanced Aesthetic Dermatology in Los Gatos, CA.

These seasoned experts share their advanced techniques and treatment protocols for addressing a wide range of challenging skin conditions in four distinct patients, including severe sun damage, flat and raised benign lesions, banding and laxity on the neck, deep peri-oral wrinkles, melasma, and hyperpigmentation.

Dr. Bitter highlights the versatility of Sciton’s fully tunable erbium YAG laser, ProFractional™, for targeting skin concerns like keratoses, sebaceous hyperplasia, and actinic keratoses. He recommends stacking ProFractional with Contour TRL™, Sciton’s full-field resurfacing treatment, or with HALO, Sciton’s hybrid fractional laser, for exceptional outcomes in patients with deeper wrinkles.

For those seeking a non-invasive, no-downtime option or those not yet ready for surgical procedures, Dr. Bitter suggests a combination of BBL® HERO™ followed by SkinTyte™.

SkinTyte™, a treatment module powered by the BBL (BroadBand light) system, improves the appearance of skin laxity on the face, neck, and body to a degree previously unseen with traditional IPL. SkinTyte can be used to treat all skin types, including skin types V and VI.

BBL may also be performed at the two-week mark after an aggressive HALO treatment to address residual erythema, says Dr. Bitter.

Dr. Pozner shares his insights on combining HALO with deep resurfacing like Sciton’s MicroLaserPeel® treatment, stressing the plethora of treatment options available on Sciton’s expandable platform, the JOULE®. He cautions that more aggressive treatments will entail longer downtime, so providers must balance the choice of treatments with patient expectations.

Dr. Bitter and Dr. Pozner highlight the beauty of Sciton’s fully tunable system, empowering practitioners to finesse the art of resurfacing, stack treatments on Sciton’s mJOULE and JOULE platforms, and deliver exceptional results tailored to each patient’s unique skin concerns.

S1 Ep3: Scars and Advanced Facial Aging

In the third episode of LaserChat, season 1, two distinguished experts, Dr. Jay Burns, MD, a board-certified plastic and reconstructive surgeon in Dallas, TX, and Dr. Jill Waibel, MD, a board-certified dermatologist in Miami, FL, share their unique perspectives and treatment techniques for tackling the most challenging aesthetic concerns explored so far in the series.

Learn how these experts utilize an arsenal of tools, including full-field and fractional laser resurfacing, pulsed light, injectables, and surgical techniques to achieve jaw-dropping outcomes. Get the full scope here: listen to season 1, episode 3.

Dr. Waibel and Dr. Burns discuss their treatment protocols for a patient with facial aging. According to Dr. Waibel, patients often express specific concerns about issues like brown spots, but their primary goal is to look and feel their best. Dr. Waibel emphasizes the necessity of a multi-faceted approach, combining surgery, injectables, and laser treatments.

To address the signs of facial aging, such as droopy eyebrows and lost volume, Dr. Waibel recommends surgical intervention around the eyes, supplemented by hyaluronic acid filler for volume restoration and laser resurfacing to address fine lines and wrinkles. Dr. Waibel says laser resurfacing is a pivotal treatment to rejuvenate the skin.

Dr. Burns underscores the importance of offering a comprehensive range of services to retain patients and cultivate loyalty to the practice. Acknowledging the inevitability of aging, Dr. Burns recommends combination therapy to address aging skin through a combination of surgical interventions, neurotoxins for dynamic wrinkles, fillers for volume, and laser skin resurfacing for skin revitalization.

In this patient, Dr. Burns performed a facelift, used the ProFractional laser around the eyes, and Sciton’s deep resurfacing treatment, Contour TRL, to eliminate upper lip lines.

S2 Ep1: Acne Scars, Melasma, Crepey Texture, Photodamage

Dermatologists Dr. Elizabeth Tanzi, MD, director of Capital Laser & Skin Care in Chevy Chase, MD, and Dr. Sherrif Ibrahim, MD, PhD, owner of Rochester Dermatologic Surgery in Rochester, NY, share their techniques and treatment protocols for addressing common aesthetic skin concerns in six patients of varying ages and Fitzpatrick skin types.

In season 2, episode 1, learn how these experts utilize advancements in laser and light-based technology to tackle atrophic acne scars, melasma, crepey texture, and the clinical manifestations of photodamage on the face and body. See the dramatic transformations they’re achieving with BBL HERO, MOXI™, Sciton’s 1927 nm non-ablative fractional laser resurfacing treatment, and the HALO laser, among other modalities.

When discussing a case of photodamaged arms, Dr. Tanzi notes that patients often express dissatisfaction with “crepey” skin texture. To address this, she treats patients with BBL HERO, Sciton’s BroadBand light device.

In contrast to the stamping approach used by other IPLs, BBL HERO employs a gliding technique, yielding superior and uniform results with a textural improvement in the skin, says Dr. Tanzi.

Following this BBL HERO treatment, Dr. Tanzi followed up immediately with a MOXI treatment. Post-treatment, patients can expect 2-3 weeks of skin that may feel like sandpaper and appear somewhat discolored. Despite this change in skin texture and appearance, Dr. Tanzi reassures patients that this typical skin healing progression doesn’t constitute significant downtime.

Dr. Ibrahim emphasizes that BBL HERO’s key benefit is the quality of treatment. Unlike traditional IPL treatments, BBL HERO offers a safer way to deliver more energy and achieve superior results. Dr. Ibrahim notes that patients, especially those undergoing facial treatments, are more comfortable during a BBL HERO treatment than with other IPL treatments.

S2 Ep2: Dyschromia, Wrinkles, Rosacea & Acne Scars

In season 2, episode two of LaserChat, Dr. Joel Cohen, MD, a board-certified dermatologist and director of AboutSkin Dermatology and DermSurgery, and Dr. Kian Karimi, MD, a board-certified facial plastic and reconstructive surgeon and founder of Rejuva Medical Aesthetics, share the treatment protocols, techniques, and energy settings that have yielded optimal outcomes in six patients.

This episode delves into the treatment strategies for improving the appearance of facial laxity, dyschromia, wrinkles, rosacea, acne scars, and other prevalent skin concerns.

Discussing a patient with rolling acne scarring, Dr Karimi recommends ProFractional, referring to it as his go-to laser for any skin type. He suggests a treatment plan of 5-7 ProFractional treatments spaced 6-8 weeks apart.

Dr. Cohen agrees with Dr. Karimi’s advice, sharing that he performed two ProFractional treatments on this patient, resulting in a noticeable improvement in the appearance of acne scars.

To further enhance results, Dr. Cohen used ProFractional’s single spot handpiece around the perimeter of the scar, employing a subcision with an 18-inch gauge and a no-core needle in the treatment area.

S2 Ep3: Active Acne, Melasma, Leg Veins, and Laxity of the Neck

Join aesthetic experts Erin Blackwell, a licensed laser practitioner, lead trainer and owner of EnlightenU laser training academy, Tammi Schue, DNP, FNP-C, a family nurse practitioner at Altaire Clinic, and renowned dermatologist Patrick Bitter, Jr., MD, in LaserChat – season 2, episode 3. Learn these clinicians’ top secrets for banishing stubborn skin issues, such as the appearance of melasma, active acne, leg veins, and neck laxity, and witness stunning results achieved through combination treatments.

The episode highlights a patient who struggled with melasma despite trying various treatments, including IPL, microneedling, and hydroquinone. The patient was frustrated and lacked trust in achieving results.

Erin proposed a HALO treatment, focusing on improving the appearance of skin texture, pore size, brightness, lifting, and tightening. After a single HALO session, the patient expressed extreme satisfaction with her results.

While the melasma is not entirely gone, it is now under control, and the skin remains clear with the ongoing use of HALO, BBL, and MOXI treatment to manage and prevent flare-ups, says Erin.

For detailed insights into Erin’s HALO settings for melasma and her pre and post-care recommendations, and to hear Dr. Bitter and Tammi’s recommendations, tune in here.
